From hospital to home, our Rehabilitation Support team is here to help you so you can live your life well. Our trained volunteers and staff will spend time with you to make sure you get the help and support you and your family need. We do this through our group and one-to-one support in the community.

We can help you and your family in lots of ways including:

  • Support either in hospital, at home or in the community
  • A friendly chat with a Rehabilitation Support Co-ordinator or volunteer in a relaxed setting to find out what matters to you
  • Help to build your confidence and overcome anxiety
  • Support to regain or relearn skills or learn new ones
  • Help to return to activities and hobbies or try new ones
  • Support to make new friendships, find support, and share experiences and information
  • Support to return to work
  • Support to improve your physical and mental health
  • Help to communicate again through our trained communication partners
